Family Caregiver Support

Caregiver logo.bmpThe Family Caregiver Support Program plans, coordinates, and provides services and support for unpaid family or friend caregivers (helpers) of adults with functional disabilities or memory impairment. The program can provide information and assistance, training, respite care, limited legal and nutritional services and counseling and other services to help support the family/friend caregiver in providing care to a loved one.

Your first step to accessing services is to submit the Caregiver Survey to our office. Once we receive your form, a case manager will contact you to review your particular situation and explain how the program can help you take care of yourself, while taking care of someone else. You may fax, mail, or E-Mail (see below) your completed TCare Survey.

Attention for returning the Caregiver Survey via E-Mail: An E-mail we receive from you may be subject to disclosure as a public record under the Public Records Act, RCW Chapter 42.56 and Email transmission cannot be guaranteed to be secure or error free, as information could be intercepted, corrupted, lost, destroyed, arrive late or incomplete or contain viruses.

If preferred, you may call our office to request we send you an encrypted email to use for returning your completed TCare survey as an attachment in the email

Caregiver Support Centers

Please call to confirm staff availability or to schedule an appointment at our Port Orchard or North Kitsap office.  Occasionally, the offices will be closed for staff meetings, holidays, illness, weather, or when staff are meeting with caregivers in the community.
